Chinese medicine is coming! Eight prevention and treatment "protocols" for adjuvant treatment of pneumonia in novel coronavirus.

  CCTV News:After the outbreak of pneumonia in novel coronavirus, Hubei Provincial Hospital of Traditional Chinese Medicine set up a fever clinic and isolation ward, and set up an expert group to deal with the epidemic. On New Year’s Eve, the expert group formulated eight protocols, covering prevention, influenza, pneumonia and recovery treatment, based on state administration of traditional chinese medicine’s "Notice on Printing and Distributing the Diagnosis and Treatment Plan for Pneumonia Infected by novel coronavirus (Trial Third Edition)" and "Chinese Medicine Prevention and Treatment Plan for Pneumonia Infected by novel coronavirus, Hubei Province (Trial Implementation)".

  On New Year’s Eve, the Agreement on Prevention and Treatment of Pneumonia Infected by novel coronavirus in Hubei Provincial Hospital of Traditional Chinese Medicine (First Edition) was released. The agreement was formulated with reference to state administration of traditional chinese medicine’s Notice on Printing and Distributing the Diagnosis and Treatment Plan for Pneumonia Infected by novel coronavirus (Trial Third Edition) and the Prevention and Treatment Plan for Pneumonia Infected by novel coronavirus in Hubei Province (Trial), combined with the previous practical work.

  Chinese medicine experts believe that novel coronavirus’s pneumonia belongs to the category of "epidemic disease" in traditional Chinese medicine. The disease is located in the lungs because of the feeling of epidemic qi, and its basic pathogenesis features are "wind, heat, dampness, poison and blood stasis". According to the evolution process of patients’ condition, the prevention and treatment agreement scheme (first edition) was formulated for clinical reference.

  Experts stressed that the formulation of the agreement on prevention and treatment of traditional Chinese medicine is not without western medicine, but with the combination of traditional Chinese and western medicine, so as to achieve better curative effect.   1. Prevention period

  Pneumonia Prevention No.1: Radix Astragali 15g, fried atractylodes 10g, Radix Saposhnikoviae 10g, Rhizoma Osmundae 6g, Flos Lonicerae 10g, Pericarpium Citri Tangerinae 6g and Eupatorium odoratum 10g.

  Indications: prevention of viral pneumonia and influenza

  2. Influenza period

  2.1 Wind-cold attacking exterior syndrome

  Clinical manifestations: initial fever, mainly low fever, aversion to cold, headache, dry throat, sore limbs and muscles, no sweat or sweating at night.

  Examination and tongue pulse: lung CT examination was negative. The tongue is pale with thin white fur and floating pulse.

  Treatment: dispelling wind and relieving exterior syndrome, clearing away heat and toxic materials.

  Recommended prescription: Influenza No.1 (Gegen Decoction or Chaige Jieji Decoction)

  15g of kudzuvine root, 10g of ephedra, 6g of cassia twig, 15g of white peony root,

  10g of ginger, 10g of raw licorice, 10g of jujube and 20g of honeysuckle.

  15g of Radix Angelicae Dahuricae for headache, and 15g of Rhizoma Belamcandae for dry throat, itching and dry cough.

  2.2 Syndrome of Heat Poisoning Attacking Lung

  Clinical manifestations: fever, aversion to cold, dry throat pain, dry cough with little phlegm, muscle pain in limbs, fatigue and headache.

  Examination and tongue pulse: lung CT examination is negative or sporadic ground glass-like high density shadow can be seen. The tongue edge is red, the coating is thin, white or yellowish, and the pulse is floating.

  Treatment: dispelling wind and relieving exterior syndrome, clearing away heat and toxic materials.

  Recommended prescription: Flu No.2 (Yinqiao Powder plus Qingwen Baidu Powder)

  10g of honeysuckle, 10g of forsythia suspensa, 10g of Schizonepeta tenuifolia, 10g of burdock,

  10g of mint, 10g of raw licorice, 10g of lophatherum gracile, 15g of reed rhizome,

  Coptis chinensis 6g

  3. Pneumonia stage

  3.1 shaoyang with dampness syndrome (fever mainly accompanied by cough, no wheezing, chest tightness, flatulence, equivalent to the early stage of pneumonia)

  Clinical manifestations: fever, mainly in the afternoon, cold and fever, cough, no wheezing, dry mouth and bitter taste, full chest and hypochondrium, upset and nauseous, silent desire to eat and drink, fatigue, pale tongue and greasy fur.

  Examination and tongue pulse: multiple or large ground glass-like high-density shadows of both lungs can be seen on lung CT examination. The tongue is slightly red with white or yellow fur, thick and greasy, and the pulse is slippery.

  Treatment: reconciliation of shaoyang and clearing away dampness and heat.

  Recommended prescription: Pneumonia No.1 (Xiaochaihu Decoction combined with Sanren Decoction or Ganlu Xiaodu Dan)

  24g of Bupleurum, 9g of Scutellaria baicalensis Georgi, 10g of ginger, 12g of Rhizoma Fagopyri Preparata,

  15g almond, 10g cardamom, 30g coix seed, 15g bamboo leaves,

  15g of talc, 30g of Smilax glabra and 10g of raw licorice.

  3.2 Damp-heat Stagnation of Lung Syndrome

  Clinical manifestations: low or no fever, dry cough, scanty phlegm, dry throat, sore throat, lassitude, anorexia, chest tightness, epiglottis, or nausea, loose stool.

  Examination and tongue pulse: multiple or large ground glass-like high-density shadows of both lungs can be seen on lung CT examination. The tongue is pale or reddish, the tongue body is fat, accompanied by tooth marks, white or greasy fur, and the pulse is moist or slippery.

  Treatment: Eliminating dampness and detoxicating, dispersing lung qi and eliminating evil.

  Recommended prescription: Pneumonia No.2 (Maxing Yigan Decoction, Xiaoxiong Decoction, Caoguo Zhimu Decoction)

  10g of ephedra, 10g of almond, 30g of coix seed, 6g of coptis root,

  10g of Faxia, 10g of Trichosanthes, 10g of Amomum tsaoko, 10g of Anemarrhena asphodeloides,

  15g of houttuynia cordata, 10g of raw licorice and 9g of cardamom.

  3.3 Syndrome of Stagnation of Lung by Toxic Blood Stasis

  Clinical manifestations: cough, chest tightness and bloating, asthma, obvious movement, shortness of breath, thirst and irritability, red and yellow urine.

  Examination and tongue pulse: multiple or large ground-glass high-density shadows of both lungs can be seen on lung CT examination, and pulmonary consolidation or fibrosis can be seen. The tongue is purple and dark, the coating is yellow or dry or thick and greasy, and the pulse is slippery.

  Treatment: detoxifying and relieving asthma, removing blood stasis and dredging collaterals.

  Recommended prescription: Pneumonia No.3 (Baihu Decoction plus ginseng soup and Situ Decoction)

  Gypsum 30g, Anemarrhena rhizome 10g, Chinese yam 15g and American ginseng 5g.

  Rhizoma Smilacis Glabrae 30g, Radix et Rhizoma Rhei 10g, Bulbus Fritillariae Thunbergii 10g, Radix Achyranthis Bidentatae 10g.

  10g of sappan wood, 10g of Eupolyphaga Seu Steleophaga, 15g of orange collaterals and 20g of radish seeds.

  15g of prickly heat seeds and 30g of loofah sponge.

  3.4 syndrome of internal closure and external detachment

  Clinical manifestations: dizziness, irritability, burning in the chest and abdomen, cold hands and feet, shortness of breath or need for auxiliary ventilation. The tongue is purple, with yellowish brown or dry fur, and the pulse is floating and rootless.

  Treatment: open and close to remove the solid, detoxify and save the inverse.

  Recommended prescription: Pneumonia No.4 (Sini plus ginseng soup, Angong Niuhuang Pill, Zi Xue Powder)

  10g of ginseng, 10g of prepared aconite root, and angong Niuhuang pill or Zi Xue powder.

  4. Recovery period

  Clinical manifestations: no fever, dry cough, chest tightness, shortness of breath, shortness of breath when moving, dry mouth and fatigue.

  Examination and tongue pulse: lung CT examination showed that inflammation began to be absorbed and interstitial changes in the lung. The tongue is reddish, greasy or thick with fur, and the pulse is thready.

  Treatment: invigorating qi and nourishing yin, tonifying lung and dredging collaterals.

  Recommended prescription: Pneumonia No.5 (Shashen Maimendong Decoction)

  15g of Adenophora adenophora, 15g of Ophiopogon japonicus, 15g of Schisandra chinensis and 12g of ginseng.

  15g of radish seed, 15g of loofah sponge, 15g of tangerine peel and 12g of perilla seed.

  12g of Fritillaria thunbergii, 12g of almond, 15g of Scutellaria baicalensis Georgi and 10g of raw licorice.
